The Future of Palliative Care: Connections between Communities & Health Care Systems
Making the End Count is an annual end-of-life professional conference for health care providers and community members to engage in timely and cogent discussions regarding end-of-life care. This year's conference will explore how healthcare systems interface with community members at the end of life. The presenters are health experts in their respective fields and provide an interdisciplinary view on providing end-of-life care. Topics included prognosis communication, evidence-based practices for palliative care and more.
Registration fee of $100 includes all sessions, lunch, snack, professional networking, & 7.0 hours of continuing education credit.
Target Audience: Physicians, Nurses, Nurse Practitioners, Physician Assistants, Pharmacists, Social Workers, Psychologists, Mental Health Counselors, Hospice professionals, Nutritionists/Dietitians, Chaplains and Community Members.
